[MEI-L] Encoding staff lines

It seems to me that what you are trying to achieve can easily be done with
the page-based customisation we use in Verovio. You can find some
information about it here http://www.verovio.org/structure.xhtml

Basically the page and the system are turned from milestone elements (<pb/>
and <sb/> respectively) to containers (<page> and <system>). The idea is
also to provide x / y and height / width information directly in there and
without referring to a facsimile tree. Where to go with this is open to
discussion.
