[mei-catalog-ig] Repetition of <extend>

Hi Klaus,

There's no reason not to make it repeatable within <expression>.  However, this might provide a starting point for a discussion here about repeated elements within the header generally.  For instance, what other elements would it be useful to make repeatable and why?
